Online registration requires Acrobat Reader. If you do not have Acrobat Reader installed, or you are having issues filling out, saving and submitting the form, please install the latest version of Acrobat Reader by clicking the following link:

Instructions (Please Print Now)

1. Open the OMH Online Registration 2013-2014 form
2. Read the instructions on the form
3. Fill out ALL required fields on the form (fields highlighted in red)
4. Click the SAVE button

After you click the SAVE button, you will be prompted to SAVE the completed form. Save the form in a location on your computer that you can easily find like DESKTOP or MY DOCUMENTS. Completed forms should then be emailed as attachments with the following ‘TO’ and ‘SUBJECT’ fields:

TO:     omh.registration@rogers.com
SUBJECT:     OMH Registration

If you have properly submitted a completed registration form as an attachment to omh.registration@rogers.com, you should receive a confirmation email. If you do not receive a confirmation email shortly after submitting your form, please contact Lynn Jennings at repteams@oshawahockey.com. Please include in the email your Child’s name and date of birth.
