An email will be automatically generated/sent to Oshawa Minor Hockey Development Program Coordinator. You will also receive a copy of this form sent to the email address supplied above (** please keep a copy **). The Development Program Coordinator will verify content of your form and contact you only if there are issues. You will be contacted prior to tryouts with more information.
When you attend your first tryout you will receive a unique 'Code' from the person that you check in with. It is important to keep this 'Code' as it will be used for player identification purposes. After receiving your 'Code' on the first night, please visit the teams webpage and confirm your 'Code' is in the list.
A Schedule will also be published on our website, please check for your start date and time.
Budget (cost) for Season will be provided at tryouts.
