This form is used for teams to submit their tournament “Wish List” and “Special Event Requests” (i.e. dates for dances and / or other fundraising events). The form will automatically be sent to Oshawa MHA General Manager.
Important Dates
* this form MUST be submitted prior to June 10.
* no approvals for your tournaments will occur until after June 10.
Guidelines
* teams will be permitted to enter three tournaments including the Oshawa tournament if your age group and division are offered.
* teams may not participate in more than 1 tournament in a calendar month
* for tournaments out of province or Canada you must submit a budget and all parents must approve.
* no teams will be permitted to enter a tournament while they are competing in the playoffs.
* Only a maximum of ten (10) teams will be approved for tournaments per weekend. If, after all scheduling meetings have taken place, there is room for more teams to be permitted on certain weekends; Oshawa MHA will revisit those requests that were not permitted.
Approvals
* If approval for your tournaments has been granted it will appear on your team calendar June 26.
After Approval has been Received
* Ensure that you have confirmation from the Host Centre of your tournament that you have been accepted (i.e. this may be closer to September).
* Submit the "Request to Enter Tournament" form (located on website under Resources / Online Forms).
*** Please remember that just because you have placed a tournament on your wish list, it does not automatically make your team available for that tournament. Until you have heard from the General Manager no out-of-town tournaments are approved. ***
